Scheduling Your Intake Appointment

New patients wishing to establish care at the Bluegrass Care Clinic should call 859-323-1688. Our Patient Service Coordinators will work with you to schedule your initial intake appointment. 

What to Expect at Your Initial Intake Appointment

When you first visit our office, you will meet with a Patient Services Coordinator (PSC) and a Care Coordinator for an intake. During this appointment they will ask questions about your past medical and social history, and will order blood tests at UK’s on-site laboratory. You may have done some of these tests in the past, but it is important to do them again so they become part of your permanent record at our office. All of the information discussed at your intake appointment is confidential, and is necessary in order to begin your care at our clinic. This information and lab work will help your doctor give you the best care possible. Your first visit usually takes about an hour and a half. Please arrive 30 minutes early for registration.

The Bluegrass Care Clinic is located on the 5th floor of the Kentucky Clinic at 740 South Limestone Street. Park on the 5th floor of the Kentucky Clinic parking garage. Turn right after entering the building and follow the signs to the Bluegrass Care Clinic. The front desk staff will register you for your appointment.

 During the intake, a PSC and a Care Coordinator will go over the following topics:

  • Your medical and social history
  • Release of previous medical records
  • Establishing care with one of our doctors
  • Filling out paperwork and disclosure forms
  • Getting your lab work done before your first doctor’s appointment
  • Making your first appointment with an HIV doctor
  • Establishing care with the Care Coordination program
  • Financial Assistance
  • Any of your other questions or concerns

Following your intake, you will be sent to the lab have several blood tests done. Your doctor will use the results from these tests at your first visit.

No one at the Bluegrass Care Clinic will be turned away because of an inability to pay. One of our financial counselors will meet with you during your intake appointment. They can help determine if you qualify for any available insurance and/or funding through our Ryan White federal grant.
